Taste and Smell Alterations and their Impact on Swallowing, Oral Intake, and Quality of Life in Head and Neck Cancer Patients

Swallowing is a complex physiological process involving coordinated oral motor and sensory functions. In patients with head and neck cancer (HNC), chemoradiation therapy (CRT) often disrupts taste and smell perception, adversely affecting swallowing, oral intake, and quality of life (QoL). This study aimed to explore the impact of taste and smell changes (TSC) on swallowing, oral intake, and QoL in individuals with HNC undergoing CRT. The Taste and Smell Questionnaire (TSQ) was administered to 97 patients treated with CRT for HNC. Swallowing, oral intake, and QoL were evaluated using the Eating Assessment Tool-10 (EAT-10), Functional Oral Intake Scale (FOIS), and Functional Assessment of Cancer Therapy–Head and Neck (FACT-HN), respectively. TSC was self-reported by 33% of participants, while 67% did not report changes. Those with TSC showed significantly higher TSQ, EAT-10, and FACT-HN scores, indicating greater perceived sensory disruption and swallowing-related impact. FOIS scores were lower in the TSC group, reflecting more restricted oral intake. TSQ demonstrated a weak positive correlation with EAT-10 (r = .29) and FACT-HN (r = .25), and a weak negative correlation with FOIS (r = –.21). These findings highlight the prevalence and impact of TSC in individuals with HNC undergoing CRT and are linked to altered swallowing experiences, reduced oral intake, and changes in perceived QoL. Although the associations with swallowing, oral intake, and QoL were weak, the results underscore the importance of assessing sensory alterations as part of routine screening and comprehensive cancer care to support nutritional status and improve overall patient well-being, thereby supporting holistic rehabilitation planning in this population.
