<p>Hydrodissection is a surgical procedure consisting in the infiltration of a fluid solution between two tissues and transforming the virtual space interposed in a real space. Hydrodissection has been used for years in several surgical procedures but no reference in literature is about its use in parotid surgical approach. In our Maxillofacial Unit of G. Martino University Hospital of Messina we routinely used Hydrodissection for surgical approach to benign tumors of the parotid gland sine 2014 and, it’s our opinion, it allows to reduce the risk of several complications as sialocele, salivary fistula, Frey’s syndrome and Facial Nerve dysfunctions but, above all, it’s related to better aesthetic outcomes. In this work we describe the application of Hydrodissection to the surgical approach of parotid gland for the treatment of benign tumors. The procedure consists in the infiltration of a solution, preferably Mepivacaina with Adrenaline 1:100.000 (alternatively Saline Solution 0.9%) into the subcutaneous tissue so that the virtual space interposed between superficial fat plane and the SMAS plane below becomes a real space. That allows an easier bluntly dissection of the subcutaneous plane and, consequently, an optimal preparation of the skin flap. In our experience, 10&#xa0;years long, Hydrodissection has been associated to a lower risk of the common complications related to the surgical approach to the parotid gland, among which the FN paralysis. However the best advantage we can report is the better aesthetic outcome thanks to a significantly lower tension of the skin flap after the final suture.</p>
