<p>Pilomatricoma is a benign subcutaneous tumor originating from hair follicle matrix cells. It typically presents as a firm, asymptomatic subcutaneous nodule with a bluish-red discoloration of the overlying skin. Although commonly located on the head, neck, and upper extremities in paediatric patients, its occurrence in the preauricular region is rare and presents as a diagnostic challenge. We report a case involving a young man with a preauricular swelling present since childhood. Clinical examination revealed a 3 × 1 cm bluish nodule in the right preauricular area. Excisional biopsy confirmed the diagnosis of ossifying pilomatricoma. The patient experienced an uneventful recovery following the excision, with no recurrence noted after a three-year follow-up. The preoperative diagnosis of pilomatricoma in the preauricular region can be particularly difficult due to limited clinician awareness. Therefore, it should be considered in the differential diagnosis of any preauricular nodular mass.</p>
