Hyper Baric Oxygen Therapy (HBOT) in ENT: A Therapeutic Option for Chondronecrosis and Osteoradionecrosis of Mixed Etiology in Head and Neck

Radiation therapy indicated for advanced-stage head and neck malignancies is implicated in the necrosis of the bony and cartilaginous skeleton of the region. Osteoradionecrosis (ORN) in the head and neck region is a delayed effect of RT exposure caused by loss of bony vascular supply and fibro-atrophy of the mandible. Laryngeal chondronecrosis is also a rare consequence of prolonged endotracheal intubation. Management techniques for such cases can range from conservative management to radical surgical intervention. A description of 4 cases in which head and neck necrotic wounds managed by HBOT has been presented in which 3 cases have necrotic wounds developed secondary to post-operative radiotherapy received for head and neck malignancy and one case is a rare case of cricoid chondronecrosis developed after traumatic intubation. After evaluation by a marine medicine physician the patients were planned for HBOT and outcomes were observed. HBOT has been reported in literature as an effective treatment modality for chronic necrotic wounds. The beneficial effects of HBOT for head and neck necrotic wounds have been discussed in the present case series.
