<p>Rehabilitation of post-COVID mucormycosis maxillectomy defects presents considerable challenges. Autogenous grafts are regarded as the gold standard for reconstruction but are often associated with unpredictable resorption and donor site morbidity. Advances in additive manufacturing technology have significantly impacted the biomedical field, leading to the use of patient-specific implants (PSI) in maxillofacial defect repairs. Zygomatic implants are also used as graftless alternative for rehabilitating post-maxillectomy defects, whether congenital or acquired. This study was designed to compare the efficacy of PSI and ZI in rehabilitating post-COVID mucormycosis maxillectomy defects. A total of 20 patients participated in the study, equally divided into Group A (rehabilitated with PSI) and Group B (rehabilitated with ZI). The results indicated that the operating time and blood loss were higher in the PSI group. However, there was no significant difference between the groups in terms of masticatory function, food leakage, or speech difficulty. Both groups showed significant aesthetic improvements. Minor complications were observed in each group but were successfully managed. No loosening of implant was observed in either group. The use of PSI and ZI in maxillectomy rehabilitation significantly reduced treatment time, restored functionality, and improved aesthetics, leading to a better quality of life for patients.</p>
