<p>Soft-tissue sarcomas (STS) are rare tumors that account for 1% of all adult malignancies, with over 100 different histologic subtypes occurring predominately in the trunk, extremity, and retroperitoneum. Alveolar soft part sarcoma (ASPS) is a rare form of soft-tissue sarcoma arising from connective tissue. It is often seen in adolescents and young adults and has high propensity for recurrence and metastasis. Clinically, it mimics hemangioma or arteriovenous malformations.</p><p>In our case, a 23&#xa0;year-old female presented with 2&#xa0;months history of foreign body sensation in throat, muffled voice, dysphagia and occasional moderate amount of hemoptysis. Per oral examination revealed left tonsil vascular mass. Computer Tomography (CT) Neck revealed a vascular mass arising from left tonsillar fossa. Histopathology post surgical excision revealed the mass to be alveolar soft part sarcoma (ASPS). CT staging and Magnetic Resonance (MR) Neck 3&#xa0;months following surgical excision showed no residual tumour or distant metastases.</p><p>The occurrence, radiological investigation and management of this disease is discussed.</p><p>We are reporting this first case of oropharynx alveolar soft part sarcoma in Malaysia due to rarity of its occurrence.</p>
