<p>Giant osteomas of the ethmoid sinus are exceptionally rare, with only a handful of cases documented in medical literature, there exists controversy surrounding the clinical representation and their optimal treatment approaches. Some cases show no symptoms, while others mimic sinus infections. Complications like chronic sinusitis and orbital cellulitis are common, highlighting the importance of understanding growth patterns and potential issues with these benign tumors. we present the case of a 52-year-old patient who had a huge ethmoidal osteoma extending into the orbit, revealed by exophthalmos and oculomotor paralysis, the tumor was successfully removed using an external approach combined with intranasal endoscopy, in this article we discuss the different surgical steps of this technique, as well as a review of the literature on this pathology, the place of the external approach, its pros and cons in comparison with other techniques.</p>
