<p>Thornwaldt’s cyst (TC) is a rare nasopharyngeal lesion that develops from primitive notochord residues. The aim of case presentation is to focus upon the advantages of endoscopic combined (transoral and transnasal) approach as well as the benefit of coblation assisted excision. A 41-year-old lady referred to ear, nose and throat outpatient department because of incidental finding of nasopharyngeal cyst after magnetic resonance imaging (MRI) due to headache. Nasal endoscopy showed benign looking midline cyst in the postnasal space, MRI suggestive of TC. Under general anesthesia, and through transoral and transnasal approaches, endoscopic coblation assisted excision was performed successfully. The patient discharge from the hospital after 24&#xa0;h, follow-up after 9 months with nasopharngoscopy showed clear cyst bed with no residual part of the cyst or infection. Transoral and transnasal endoscopic excision of TC were easy and accessible and provide proper visualization of the cyst with complete removal especially with the use of coblation assisted resection, coblation provided easy and satisfactory dissection with good hemostasis.</p>
