Evaluating the Efficacy of Alpha-Lipoic Acid Combined with Intralesional Steroids in OSMF

The objective of this study was to compare the clinical efficacy of lipoic acid in combination with intralesional triamcinolone acetonide versus intralesional triamcinolone acetonide alone in the management of Grade II Oral Submucous Fibrosis (OSMF). This was a prospective, randomized clinical trial conducted on 30 patients diagnosed with Grade II OSMF based on clinical examination. The participants were randomly assigned into two groups: Group A (n = 15) received treatment with intralesional triamcinolone acetonide alone, while Group B (n = 15) received a combination of intralesional triamcinolone acetonide and oral lipoic acid. Both treatments were administered over 12 weeks. The efficacy of the treatments was evaluated at four intervals: 3 weeks, 6 weeks, 3 months, and 6 months post-treatment. The parameters assessed included the following: Burning sensation in the mouth, Blanching of oral mucosa, Presence of vesicles or ulcerations in the oral cavity, Mouth opening (inter-incisal distance). At the end of the study, Group B demonstrated significantly better outcomes in terms of increased mouth opening and a greater reduction in the burning sensation in the oral cavity compared to Group A. Both groups showed improvements in the blanching of mucosa, but the rate of improvement was more pronounced in Group B. The combination of lipoic acid and triamcinolone acetonide was well-tolerated, with no significant adverse effects reported during the treatment period or follow-up. The addition of lipoic acid to intralesional triamcinolone acetonide therapy resulted in a more favourable clinical response in terms of mouth opening and reduction in burning sensation, compared to triamcinolone acetonide alone. These findings suggest that the combination therapy may offer an enhanced therapeutic effect in managing Grade II OSMF. However, further studies with larger sample sizes and extended follow-up periods are necessary to confirm these results and establish this combined treatment approach’s long-term efficacy and safety.
