An Experience of Team in Endoscopic Trans Nasal Trans Sphenoidal (TNTS) Pituitary Surgery: A Case Series from Single Specialty Centre

This study is aimed to analyse surgeons perspective of intra operative exposure, clarity of field, learning curve in operative time and reconstruction during endoscopic trans nasal trans sphenoidal pituitary surgery (TNTS). Patients of sellar lesions and extension who underwent endoscopic TNTS were retrospectively evaluated. Details of demography, clinical, radiological, intraoperative- postoperative findings, histopathological features were extracted from clinical case records. In addition, the Otorhinolaryngologist and Neurosurgeon opinion on exposure of the lesion, visualisation of intra operative clarity and operative duration captured were analysed. The learning curve of team was assessed as operative time required and successful reconstruction. A total of 12 sellar lesion posted for endoscopic TNTS during the study period. Seven participants were males while five were females. Four patients had visual field defects. In 10 cases the exposure was perceived as above average, while clarity of the field was perceived as above average in 11. Mean (SD) duration of surgery in hours was reduced in second phase cases 1.58 (0.20) compared to first phase 2.83 (0.51). All 12 cases had successful reconstruction of the defect. Fat alone was used in nine cases for reconstruction. The exposure, clarity of vision and operative timings improved over period of time as team worked together. Sellar lesions after careful removal by endoscopic TNTS can be safely closed by free soft tissue using fat graft.
