<p>This case report describes a rare anatomical variation of the chorda tympani nerve encountered during a Transcanal Endoscopic Tympanoplasty. The nerve was found lateral to the tympanic sulcus, embedded within the tympanomeatal flap, and exited the middle ear without a bony canal, coursing toward the malleus handle. This atypical presentation deviates from established classifications and poses a unique surgical challenge in preserving the integrity of the nerve. Careful dissection and meticulous surgical planning are essential to preserve chorda tympani function and to prevent postoperative complications.</p>
