<p>Rhabdomyoma is a rare benign neoplasm originating from skeletal muscle. While it commonly affects the head and neck region among extracardiac sites, solitary lingual adult rhabdomyomas (ARs) are extremely uncommon, with few reported cases in the literature. We present a 39-year-old male who presented with a 2-month history of globus sensation, dysphonia, shortness of breath, snoring and apneic episodes at night prompting evaluation in the ENT OPD. Fine needle aspiration cytology (FNAC) of the lingual swelling revealed rhabdomyoma. The patient underwent successful complete endoscopy assisted intraoral excision of the lesion, underscoring the importance of meticulous surgical management for large rhabdomyomas. This case highlights the diagnostic challenges and therapeutic considerations in managing solitary lingual adult rhabdomyomas.</p>
