<p>Pulmonary valve (PV) infective endocarditis is a rare condition with limited data available on optimal surgical approaches. We present a unique case of a 47-year-old male with fungal PV endocarditis, managed successfully with a single-leaflet autologous pericardial reconstruction using a modified Ozaki technique. The procedure demonstrated technical feasibility and favorable early postoperative outcomes, offering an alternative to conventional prosthetic valve replacement in selected patients.</p>
