<p>A 74-year-old Asian female patient with a history of hypertension and dyslipidemia presented with a subacute dry cough of&#xa0;one month&#xa0;duration.&#xa0;Chest radiography revealed mediastinal enlargement and contrast-enhanced computed tomography revealed a huge saccular aortic arch aneurysm compressing the left lung and protruding to the anterior chest wall. Open surgical intervention using a frozen elephant trunk was successfully performed to treat the aortic aneurysm, and the cough improved completely with aneurysm sac shrinkage six months postoperatively. This case report describes an uncommon presentation of thoracic aortic aneurysm and its outcomes following surgical intervention.</p>
