Rupture of splenic artery aneurysm during early pregnancy: two-autopsic cases

Splenic artery aneurysm (SAA) is an uncommon and life-threatening vascular disease. This pathology is frequently diagnosed during pregnancy. The major complication of this aneurysm is rupture, which happen frequently in pregnant women due to physiological changes. We report, two rare cases of sudden death due to SAA rupture occurring during the first two trimesters of pregnancy and discovered during autopsy. Case 1: A 26-weeks pregnant woman consulted the Emergency for a sudden onset of abdominal pain and dizziness. An ultrasound showed fetal death. She underwent two surgeries, and the origin of bleeding still could not be seen. The patient was declared dead the same night. A medico-legal autopsy was requested to determine the cause of death. On autopsy, a ruptured SAA was found to be the cause of death. Case 2: An 8-weeks pregnant woman presented sudden abdominal pain with a rapid loss of consciousness. She was transferred to a clinic where she arrived dead. A medico-legal autopsy was required, that showed a saccular aneurysm of distal segment of the splenic artery that ruptured into the peritoneum causing an important hemorrhage. The cause of death was a hemorrhagic shock due to SAA rupture. SAA is a rare entity that can be responsible for sudden death. This risk is clearly higher during pregnancy with maternal and foetal compromise. These two cases illustrate a rare event of previously undocumented SAA in pregnant women that ruptured during early pregnancy and first diagnosed at autopsy.
