This review aims to examine fatty liver in pregnancy, specifically acute fatty liver of pregnancy (AFLP) and metabolic dysfunction-associated steatotic liver disease (MASLD). It seeks to clarify their distinct pathophysiology, clinical features, diagnostic challenges, and management strategies, while identifying gaps in current knowledge and practice.
Recent FindingsEmerging data highlight the role of mitochondrial dysfunction and genetic mutations, such as LCHAD deficiency, in AFLP, and the contribution of insulin resistance and metabolic syndrome in MASLD. Both conditions are linked to serious maternal and fetal outcomes. Advances in imaging, multidisciplinary management, and lifestyle interventions have improved diagnosis and care, but standardized protocols are still lacking.
