Purpose of Review <p>This review aims to examine fatty liver in pregnancy, specifically acute fatty liver of pregnancy (AFLP) and metabolic dysfunction-associated steatotic liver disease (MASLD). It seeks to clarify their distinct pathophysiology, clinical features, diagnostic challenges, and management strategies, while identifying gaps in current knowledge and practice.</p> Recent Findings <p>Emerging data highlight the role of mitochondrial dysfunction and genetic mutations, such as LCHAD deficiency, in AFLP, and the contribution of insulin resistance and metabolic syndrome in MASLD. Both conditions are linked to serious maternal and fetal outcomes. Advances in imaging, multidisciplinary management, and lifestyle interventions have improved diagnosis and care, but standardized protocols are still lacking.</p> Summary <p>AFLP and MASLD in pregnancy pose significant risks. Early recognition and multidisciplinary care are crucial. Future research should focus on genetic screening, long-term outcomes, and the development of unified clinical guidelines to enhance maternal-fetal health and inform preventive strategies.</p>

This review aims to examine fatty liver in pregnancy, specifically acute fatty liver of pregnancy (AFLP) and metabolic dysfunction-associated steatotic liver disease (MASLD).
