<p>Internal mammary artery (IMA) injury resulting from blunt trauma is a rare but potentially life-threatening condition. We present the case of a 44-year-old man who sustained multiple injuries, including a sternal fracture, after a fall from a walnut tree. The sternal fracture resulted in a left IMA injury, leading to an anterior mediastinal hematoma and extrapericardial tamponade. Despite prompt cardiopulmonary resuscitation, emergency surgery including ligation of the injured artery, the patient died on the third postoperative day due to multiorgan failure. This case highlights the importance of early recognition and urgent surgical intervention in managing IMA injuries to prevent fatal outcomes.</p>
