<p>Aggressive behavior in acute psychotic patients remains partly unavoidable despite reforms following the Psychiatry Enquete promoting more humane and socially oriented psychiatric care. Therefore, differentiated structural, milieu-therapeutic and interinstitutional approaches are required to prevent aggression and ensure the safety of patients, co-patients and staff in acute psychiatric settings. In 2018, our acute psychiatric ward was comprehensively redesigned and transformed into a unit incorporating Soteria elements. A small protected unit with 6 beds merges into an open unit with 15 beds. The open setting provides a warm, low-stimulus and homelike therapeutic environment with a structured, everyday-oriented program including group and social activities. Care is delivered by a multiprofessional team specifically trained in de-escalation and interpersonal communication, working consistently in a relationship-oriented manner. A pre–post comparison showed a significant reduction in aggressive incidents, longer treatment duration in the open setting and improved continuity of care within the psychiatric system. The concept also proved effective in patients with dual diagnoses and complex psychosocial needs. An additional preventive measure is the “Round Table Psychiatry–Police Oberhavel”, established in 2022. Quarterly meetings involve representatives of acute psychiatry, the police, social psychiatric services, guardianship courts and emergency medical services. Complex cases are discussed anonymously to develop practical, case-oriented solutions. Improved interdisciplinary understanding and optimized interfaces contribute further to reducing aggression and violence in acute psychiatric care.</p>
