LUPS, A Novel Medium with Enhanced Diagnostic Sensitivity for Trichomonas vaginalis in Comparison with PEHPS and TYI-S-33.

According to the WHO, in 2020, trichomoniasis affected 156 million people aged between 15 and 49 years. Trichomoniasis has been associated with preterm birth, low birth weight, newborn infection transmission, cervical cancer, an increased risk of prostate cancer, and increased HIV acquisition. This study aims to investigate the diagnostic performance of LUPS media compared to wet mount, and culture with TYI-S-33 and PHEPS media.
MethodsA cross-sectional observational study was carried out. A total of 120 vaginal exudate samples were collected from adult women from Nuevo Leon, Mexico, from March to November 2019. Samples were analyzed for T. vaginalis presence by wet mount, culture with LUPS and PEHPS media (not commercially available), TYI-S-33 media and PCR. Growth rate and doubling time were assessed on GT-15 cells. Diagnostic tests, and ROC curves were performed.
ResultsAfter 48 and 72 h of culture, increased growth (p < 0.05) was observed with LUPS compared with TYI-S-33. Positivity was 0.8% (n = 1), 18.3% (n = 22), 19.2% (n = 23) and 21.7% (n = 26) for wet mount, TYI-S-33 and PEHPS, LUPS and PCR, respectively. An AUC of 0.519, 0.885 and 0.942 were obtained for wet mount, TYI-S-33 and PEHPS, and LUPS respectively. The highest sensitivity was observed with LUPS (88.5%) followed by TYI-S-33 and PEHPS (76.9%) and wet mount (3.9%).
ConclusionsTherefore, we propose that culture systems should be implemented at the IMSS, which detects 84.7% more patients compared to wet mount when using LUPS media. The economic impact that trichomoniasis represents must be considered.
