Rotator cuff repair aims to reduce pain and restore function. Besides surgery itself, several patient-related factors can influence clinical outcomes. Among these, nutrition has gained growing interest. This review investigates the current evidence regarding the role of macro- and micronutrients, as well as selected supplements on tendon healing following rotator cuff repair. While protein intake, essential amino acids, and vitamin D may support immune function, muscle maintenance, and reduce retear risk, their direct effect on tendon healing remains unclear. Although general supplementation cannot be recommended at this time, an individualized, protein-rich diet—potentially combined with targeted supplementation in high-risk patients—may positively influence postoperative recovery.
