<p>The case of a&#xa0;61-year-old man after elective reverse shoulder arthroplasty in another hospital with clinical signs of an acute periprosthetic infection is presented. As the wound condition did not improve, the patient was referred to our institution and the anti-infective therapy was continued. Due to deteriorating wound conditions and after interdisciplinary case evaluation, the diagnosis of exclusion of pyoderma gangrenosum was made. After initiation of appropriate therapy with high-dose prednisolone, healing was achieved followed by soft tissue coverage with a&#xa0;split thickness skin graft.</p>
