<p>This paper presents the results of a failure analysis of a plate core in a plate-and-shell heat exchanger (PSHE). To investigate the cause of cracking, the following methods were employed: a background investigation, macro-examination, analysis of the chemical composition, quantitative assessment of wall thinning and crack observation, metallographic examination, scanning electron microscopy (SEM) analysis, and finite element stress analysis. The results revealed significant mechanical damage in the outer circumferential weld area of the plate core. This damage resulted in wall thinning and structural discontinuity in the core. Under fatigue loading, cracks initiated simultaneously from both the inner and outer surfaces, leading to core leakage.</p>
