On the Importance of Good Surface Quality in Implants: A Failure Analysis Case as Bad Surface Quality Example

The increase of surgical orthopedic implant use in the human body in the last decades, due to the population aging and higher exposure to the risk of several types of accidents, has enhanced the importance of study of the performance of these materials in such applications, mainly preponderant factors such as chemical composition, microstructure, and characteristics of manufacture, handling, and use of these devices. In this context, the knowledge about characteristics/properties of these materials is very important. Among these characteristics, a bad surface quality can considerably harm the performance of an orthopedic implant. As an example, in this work, a microstructural and fractographic study on the failure analysis of an orthopedic surgical implant is presented. This implant, manufactured with stainless steel, was inserted as a femoral prosthesis in a young victim of an accident. The results demonstrate that, although the stainless steel used in the manufacture of the implant met the specified requirements for chemical composition and microstructure, the marks left on the surface, probably by installation/handling or by inappropriate use of the implant, acted as stress raisers, which, in conditions of natural cyclic loading, favored the initiation of fatigue fracture that led to the material failure.
