The introduction of molecular markers has significantly improved the prognosis and treatment of endometrial cancer in recent years. As endometrial cancer is usually diagnosed in early stages, surgery is the most important therapeutic aspect and comprises hysterectomy with bilateral adnexectomy. In case of additional risk factors, sentinel node biopsy is recommended. For advanced stages, systematic pelvic and paraaortic lymphadenectomy should also be performed. After surgery, adjuvant therapy should be considered, particularly for patients at a high risk of recurrence. This may include adjuvant radio- and/or systemic therapy, with immunotherapy emerging as a key component of the treatment regimen. In cases of recurrence, endocrine therapy may also be an option. Overall, the treatment of endometrial cancer has become increasingly complex and should be managed in an interdisciplinary approach at a certified gynecologic cancer center.
