Diabetic polyneuropathy is one of the most common chronic complications of diabetes with a major impact on mortality and quality of life, yet it remains a frequently overlooked and undertreated condition in clinical practice. It is typically characterized by a progressive distal to proximal loss of sensation in the lower extremity and, in around 50% of cases, with neuropathic symptoms in this area. A careful exclusion of other causes and efficient standardized clinical assessments aid in screening and a timely diagnosis. Strategies for prevention and treatment include the reduction of risk factors, pathogenetically-oriented approaches, and symptomatic management of neuropathic pain. Various mono- and combination pharmacotherapies as well as topical and neuromodulatory techniques are used to improve not only pain intensity but also sleep and quality of life.
