<p>Intensive pesticide use in the province of Larache, Morocco, is leading to groundwater contamination and posing risks to human health and the environment. However, previous work has been largely limited to pesticide monitoring, without any in-depth assessment of the associated risks. The aim of this study is to assess the level of pesticide contamination in groundwater and to estimate the associated health (carcinogenic and non-carcinogenic) and ecological risks. To assess these risks, we collected 11 groundwater samples in the province of Larache (Morocco) in June and July 2023, through spot sampling in two intensively farmed areas, Souaken and Aouamra, from wells located on farms. Pesticides were extracted using the liquid–liquid extraction method, then analyzed by gas chromatography coupled with an electron capture detector (GC-ECD). The results revealed the presence of 16 active substances in groundwater, several of which exceeded national (ONSSA, N.M. 03.7.001) and European (Directive 98/83/EC) regulatory limits set at 0.1&#xa0;µg/L, with maximum concentrations reaching 11.023&#xa0;µg/L for dichlorvos. The health risk assessment showed that dichlorvos, fenamiphos, triadimenol, cadusafos, and fenarimol present high health risks, particularly for infants and children, highlighting the potential for chronic exposure that may lead to long-term adverse health effects. Furthermore, the ecological risk analysis revealed that dichlorvos, fenamiphos, and cadusafos pose a high ecotoxicological risk. These substances are likely to impair essential ecological functions of groundwater aquifers, such as the regulation of biogeochemical cycles and the maintenance of water quality. These results highlight the diffuse contamination of groundwater by pesticides, with major implications for public health and the environment. They are intended to support policymakers, health authorities, researchers, and agricultural stakeholders in implementing risk management and water resource protection strategies, as well as in identifying priority substances and areas for monitoring.</p>
