<p>Globally, attention has been drawn to the increasing occurrence of antimicrobial resistance genes (ARGs) in wastewater, which is now recognized as an important reservoir for the dissemination of antimicrobial resistance (AMR). Yet, the research landscape for AMR in wastewater is not fully understood. Thus, there is an urgent need for a bibliometric analysis to assess trends and gaps in AMR research in wastewater. This study utilised the PubMed database to retrieve relevant datasets that were analysed using diverse tools, including Biblioshiny, RStudio and VOSviewer. A total of 3,462 records were retained as part of the final dataset. Year-wise analysis of the documents revealed two phases of growth, including a stagnation phase that spanned from 1978–2010 (1–25 articles/year), and a rapid growth phase from 2011–2025 (34–615 articles/year). Original studies dominated (89%), followed by reviews (10.9%) and systematic reviews (0.8%). Keywords analysis revealed five major research clusters or themes, including clinical antimicrobial resistance, wastewater and environmental surveillance, One Health intersections, methodological innovations, and antibiotic-resistant bacteria. Temporal analysis identified emerging themes, including COVID-19, genomics, phage therapy, carbapenemases, and mobile genetic elements. The top three most prolific authors, Zhang Tong, Amy Pruden, and Gao Jingfeng, collectively produced 114 documents. Top dissemination outlets were <i>Science of the Total Environment</i>, <i>Water Research</i>, and <i>Journal of Hazardous Materials</i>. Global collaborations in AMR research in wastewater were dominated by countries in Asia, Europe, and North America, while Africa, South America, and Russia remained under-represented. To ensure a balanced and comprehensive AMR research landscape, there is a need to enhance One Health approaches, elevate surveillance programmes, promote equitable international collaborations, translate research into implementable policies, and build local research capacity, especially in under-represented regions.</p>
