Validity and reliability of the Turkish version of the uremic pruritus in dialysis patients (UP-Dial) scale

Uremic pruritus significantly impairs the quality of life in hemodialysis (HD) patients, a specific assessment tool for this population is lacking. The primary purpose of this study was to translate the Uremic Pruritus in Dialysis Patients (UP-Dial) scale into Turkish and evaluate its validity and reliability.
MethodsIn this methodological study, 181 HD patients with pruritus were included. The reliability of the Turkish UP-Dial scale was tested using Cronbach’s alpha values for internal consistency. Construct validity was examined via exploratory factor analyses (EFA) and confirmatory factor analyses (CFA). Convergent validity was assessed by correlating the UP-Dial with the Numerical Rating Scale (NRS), 5-D Itch Scale, and Dermatology Life Quality Index (DLQI).
ResultsFollowing item analysis, item 7 (xerosis) was excluded due to low item–total correlation. The 13-item Turkish UP-Dial demonstrated excellent internal consistency (Cronbach’s α = 0.90) and test–retest reliability (ICC = 0.98; 95% CI = 0.97–0.99). EFA revealed a four-factor structure explaining 78.5% of the total variance, and CFA demonstrated good model fit (χ2/df = 1.97, RMSEA = 0.07, CFI = 0.97, GFI = 0.91). Strong correlations were found between UP-Dial total scores and NRS (r = 0.82), 5-D Itch Scale (r = 0.87), and DLQI (r = 0.80) (all p < 0.001).
ConclusionThe 13-item Turkish UP-Dial is a valid and reliable tool for multidimensional assessment of uremic pruritus in HD patients. It may provide standardized follow-up and evaluation of responses to both pharmacologic and non-pharmacologic treatments for uremic pruritus.
