<p>This paper constructs an evaluation index system for the healthy aging of older adults using the entropy-weighted TOPSIS method. It analyzed data from the China Health and Retirement Longitudinal Study (CHARLS) from 2011 to 2018 to assess the impact of the Healthy Cities Pilot Policy on healthy aging levels. The study revealed several key findings: (1) the Healthy Cities Pilot Policy notably enhanced healthy aging among older adults, (2) enhanced environmental livability, particularly the increase in per capita hospital beds, played a significant mediating role, and (3) the Healthy Cities Pilot Policy was more effective for older adults with compulsory education or urban residence. To maximize the effects of the Healthy Cities Pilot Policy, we recommend expanding pilot coverage and healthcare infrastructure, enhancing community engagement and public fitness facilities, and offering targeted support to rural and less-educated populations to promote equity.</p>
