<p>This work presents a high-performance terahertz reflective cross-polarization conversion metasurface with integrated biosensing capabilities. The design, based on an octagonal split-ring resonator fabricated on a gold-polyimide substrate, attains a polarization conversion ratio (PCR) surpassing 90% across the 3.80–8.25 THz range, with an average PCR of 97.25%, peak efficiency of 99.87% and sustained average efficiency of 96.25% at oblique incidences up to 20°. In addition to its polarization functionality, the proposed design operates as a biosensor capable of detecting refractive index variations linked to various cancer types, reaching a maximum sensitivity of 1.69 THz/RIU. This dual-functional approach shows the feasibility of uniting ultrawide polarization conversion and high-sensitivity biosensing in a single, cost-effective terahertz device, offering strong potential for advanced biomedical and photonic applications.</p>
