Background/Objectives <p>Intraocular lens (IOL) pupillary capture is a frequent and debilitating postoperative complication that can cause significant vision loss, discomfort, pain, and even endophthalmitis. It arises from factors such as pupil overdilation, IOL deviation, iris laxity, and reverse pupil block, leading to IOL entrapment within the pupil. Traditional management methods—including conservative observation, medication, YAG laser iridotomy, or surgical reset—have limitations such as high recurrence rates, surgical risks, and potential secondary damage. This study introduces a novel surgical technique, single suture interception, to provide a safer, more efficient solution for this challenging condition.</p> Methods <p>We intercepted and fixed the intraocular lens by using a 10–0 polypropylene suture which was threaded through the lateral incision of the corneal limbus into the iris to form two interception lines across the pupil.</p> Results <p>The single suture interception technique was used in 10 patients with intraocular lens pupillary capture, and none of them had postoperative recurrence and there were no other complications.</p> Conclusions <p>Single suture interception is an effective method for solving intraocular pupillary capture. It is simple, convenient and safe.</p>
