<p>The present investigation was performed to compile and assess of the medicinally important plants utilized through the indigenous communities of Chhattisgarh state in their traditional medical practices.The study included 58 local informants, and their data were obtained in a semi-structured interview in 2022–2023 and by questionnaires. Semi structured ethnomedicinal analysis including the Use Value (UV), Fidelity Level (FL), Informant consensus Factor (ICF), Frequency of citation (RFC), Usage Report (UR) and Family importance Value (FIV)were used as the measures of relevance and reliability pertaining to plant usage between indigenous people. Among 158 recorded species, nine are new ethnomedicinal uses, a result that greatly extends the regional pharmacopoeia. Crucially, nine plants such as <i>Anogeissus latifolia</i> (Roxb. ex DC.) Wall. ex Guill. &amp; Perr. (wound healing), <i>Urena lobata&#xa0;</i>L. (urogenic disorders) and <i>Cyanthillium cinereum (L.) H.Rob.</i>(Polycystic Ovary Syndrome) were documented for the first time elsewhere as well as described in the literature with unknown ethnomedicinal properties. The documentation of nine ethnobotanical species stands out as a major contribution of the current research, providing potential sources for pharmacological screening and integrative health care development. This paper reveals the urgent need in conservation measures, scientific authentication and cultivation programs that would preserve native medicinal plants as they are overexploited and urbanized.</p> Graphical Abstract <p></p>
