<p>Concomitant Wilkie and Nutcracker syndromes represent a rare clinical challenge, often requiring complex vascular and digestive surgical interventions. We report a unique case of refractory postoperative gastroparesis following surgical correction, successfully managed with Gastric Per-Oral Endoscopic Myotomy (G-POEM). Representing the first description of G-POEM as a rescue intervention in this complex multi-syndromic setting, our findings suggest that endoscopic pyloromyotomy provides a safe, minimally invasive alternative to revisional surgery for restoring gastric kinetics and ensuring durable symptomatic resolution.</p>
