Background and Aims <p>Low-volume, single-dose bowel preparations may improve patient convenience and compliance while maintaining cleansing quality. This study evaluated the efficacy and safety&#xa0;of a same-day, low-volume, single-dose D-sorbitol, ascorbic acid, and sodium picosulfate bowel cleansing regimen in routine outpatient practice.</p> Methods <p>From May 2022 to July 2025, a prospective, multicenter post-marketing surveillance was conducted in four centers in Korea. Adult outpatients aged 19 to 80&#xa0;years scheduled for elective colonoscopy and prescribed the study regimen were enrolled, excluding those with suspected bowel obstruction or severe comorbidities. The regimen consisted of 230&#xa0;mL of reconstituted solution followed by 460&#xa0;mL of water, administered 4–5&#xa0;h before colonoscopy. Cleansing quality was graded using the Harefield Cleansing Scale (HCS); adequate cleansing was defined as Grade A or B. Secondary outcomes were regimen completion and adverse events. All variables were summarized using descriptive statistics.</p> Results <p>A total of 608 patients (mean age 53.0 ± 12.9&#xa0;years; 49.8% male; mean BMI 23.8 ± 3.2&#xa0;kg/m<sup>2</sup>) were analyzed. Adequate cleansing was achieved in 96.4% (HCS Grade A: 68.1%, Grade B: 28.3%), with a regimen completion rate of 99.7%. Polyps were detected in 249 of 608 patients (41.0%), with a higher detection rate in men than in women (49.2% vs. 32.8%,&#xa0;<i>p</i> &lt; 0.001). Adverse events occurred in 14.6% of patients, most commonly nausea (9.9%) and vomiting (5.1%). All adverse events were mild or moderate, resolved within 24&#xa0;h, and required no hospitalization.</p> Conclusion <p>This prospective, observational study demonstrated that a same-day, low-volume D-sorbitol, ascorbic acid, and sodium picosulfate bowel cleansing regimen showed high efficacy, excellent completion rates, and a favorable safety profile in outpatient colonoscopy practice.&#xa0;This could represent a convenient alternative to traditional split-dose preparations, potentially improving patient compliance.</p>
