Unmeasured Confounders and Control Group Heterogeneity in Laser-Assisted Lead Extraction

This letter discusses methodological considerations regarding the recent study by Lynch et al. on cardiovascular implantable electronic device (CIED) lead extraction. While acknowledging the study’s valuable real-world data, we highlight three critical issues. First, the extreme heterogeneity in the control group—which combines low-risk manual traction with high-risk open surgical extraction—may obscure true clinical comparisons. Second, the omission of key local anatomical variables, such as lead calcification and adhesions, introduces significant unmeasured confounding. Finally, baseline differences in left ventricular ejection fraction and ventricular tachycardia likely reflect a higher proportion of complex dual-coil implantable cardioverter-defibrillator (ICD) leads in the laser group, warranting rigorous stratified analysis. We recommend incorporating detailed anatomical data and precise categorizations in future studies to optimize clinical extraction strategies.
