Multimodality imaging of pulmonary artery spindle cell sarcoma in Behçet’s disease with cardiac valve involvement

Cardiac valve disease secondary to Behçet’s disease complicated by pulmonary artery spindle cell sarcoma is extremely rare. We report a 59-year-old man with recurrent oral ulcers who presented with fever and chest pain. Laboratory findings and a positive pathergy test supported the diagnosis of Behçet’s disease. Echocardiography showed severe tricuspid regurgitation and a pulmonary artery mass, which was corroborated by CT and cardiac MRI. PET/CT demonstrated hypermetabolic activity, and histopathology confirmed spindle cell sarcoma. This case underscores the pivotal role of multimodality imaging in the timely diagnosis of pulmonary artery spindle cell sarcoma associated with valvular involvement in Behçet’s disease.
