<p>In a historical development, the Pakistani parliament made significant strides toward recognizing the rights of the third gender by enacting the Transgender Protection of Rights Act in 2018. This legislation represents a pivotal milestone in acknowledging and safeguarding the rights of transgender individuals in Pakistan. The present study sought to investigate the barriers encountered by transgender women in accessing gender-affirmative treatment for the purpose of gender transition. Ten transgender women participated in semi-structured and in-depth interviews, employing the grounded theory method for analysis. Analysis uncovered a spectrum of medical, social, sexual, legal, religious, financial, and psychological barriers that participants faced during the transition process. These findings are contextualized within Pakistan's cultural milieu and examined in relation to existing literature. The outcomes of this research have the potential to enlighten clinicians, academics, policymakers, and other stakeholders about the unique challenges encountered by transgender women, thereby fostering greater awareness and understanding.</p>
