Lung volume reduction surgery (LVRS) is the cornerstone of surgical treatment for chronic obstructive pulmonary disease (COPD) and results in long-term, stable functional improvement. The following overview outlines the development and current status of LVRS. The focus is on patient selection, perioperative management and the role of LVRS in relation to alternative endoscopic procedures. Finally, key aspects of lung transplantation as the last resort for end-stage COPD are discussed.
