<p>Seizures are the most frequent neurological emergency in neonates. Neonatal seizures are mostly acutely provoked but in ca. 15% they can be the primary manifestation of a&#xa0;neonatal onset epilepsy. The pharmacotherapy has to take specific physiological aspects of the neonatal central nervous system as well as pharmacokinetic and ethical issues into account. This review is based on the guidelines and recommendations for the treatment of seizures in neonates published by the International League Against Epilepsy (ILAE) in 2023, supplemented by current studies.</p>
