<p>The treatment of people with epilepsy (PWE) using antiseizure medication (ASM) is often necessary for the lifetime. The long-term effects of ASM can be multifarious and greatly depend on the type of medication, the dosage and duration of intake. The long-term effects include cognitive and psychiatric effects, such as depression and anxiety disorders, an elevated cardiovascular risk, the development of osteoporosis with an increased risk of fractures, hormonal changes, hepatotoxicity, electrolyte disorders, nephrolithiasis, body weight changes, blood abnormalities and dermatological effects. A&#xa0;detailed discussion about the possible long-term effects of ASM should take place at every consultation with the treating neurologist in order to identify and counteract undesirable side effects at an early stage of treatment. This narrative review article presents the most important known long-term effects of ASM on the nervous system, cardiovascular system, bones, liver, kidneys, electrolyte balance, skin and blood and provides strategies for monitoring and preventing unnecessary long-term sequelae caused by ASM.</p>
