<p>Between 8&#xa0;and 13% of all women of reproductive age are affected by polycystic ovary syndrome (PCOS), making it one of the most common endocrinopathies in women. In recent years, increasing attention has been paid to the long-term consequences of PCOS for metabolism and cardiovascular health, and to the need for early preventive measures. Since this can only succeed through an interdisciplinary approach, a&#xa0;new S2k guideline for the diagnosis and treatment of PCOS was developed under the leadership of the German Society for Endocrinology (DGE), with the support of the Professional Association of German Internists/Endocrinology/Diabetology Section (BDI). The treatment concept for PCOS is multimodal and should include basic measures such as behavior modification, a&#xa0;healthy diet, and physical activity. As pharmacotherapy, metformin is considered highly important and can be used off-label in women with PCOS without diagnostic testing of glucose–insulin metabolism. When choosing hormone therapy for endometrial protection, in addition to cardiovascular risks, the patient’s preferences should also be taken into account in order to achieve the greatest possible adherence. Ovarian stimulation with letrozole is the most effective first-line therapy for PCOS patients wishing to conceive.</p>
