<p>More than 400,000 women worldwide are diagnosed with endometrial carcinoma (EC) each year, with a&#xa0;steadily increasing incidence. Despite numerous advances in treatment, including the use of immune checkpoint inhibitors, endocrine therapy continues to play a&#xa0;relevant role. Following careful patient selection, it represents a&#xa0;meaningful and generally well-tolerated treatment option both for patients with limited fitness for intensive therapy and for young women seeking fertility preservation. Progestins have been used for this purpose for many years. Current research focuses on the role of local progestin administration in fertility-sparing treatment strategies, as well as on the use of aromatase inhibitors in combination with Cyclin-dependent kinase 4/6 (CDK4/6)&#xa0;inhibitors with the aim of improving response rates in recurrent EC. In addition, the potential benefit of additive therapeutic approaches, such as metformin, is being investigated.</p>
