<p>Preconception vaccination counseling is a&#xa0;key aspect of obstetrical preventive care, aiming to protect against vaccine-preventable infectious diseases during pregnancy and their potentially severe maternal and fetal complications. It includes a&#xa0;thorough assessment of individual risk factors and a&#xa0;structured review and possible optimization of the serostatus with regard to specific infections. While inactivated vaccines can be administered during pregnancy, live vaccines should be given prior to conception. Vaccinations against pertussis, influenza, SARS-CoV‑2, and respiratory syncytial virus (RSV) primarily aim to protect the newborn through passive immunity. Counseling must address vaccination history, serological testing, appropriate timing, and contraindications. Evidence-based recommendations, such as those from the Standing Committee on Vaccination (STIKO) in Germany or the national vaccination committees in Austria and Switzerland, are updated annually. The goal of comprehensive preconception prevention is to ensure optimal protection for both mother and child.</p>
