<p>Menstrual cycle-linked symptoms of a premenstrual syndrome (PMS) are routine gynecological problems, whereas cycle-dependent symptom fluctuations or premenstrual exacerbations (PME) in mental diseases, such as depression or psychotic disorders, are less frequent reasons for treatment in gynecological practices. In cases of fluctuations of the mental symptoms during the menstrual cycle the differential diagnostics of a PMS or premenstrual dysphoric disorder (PMDD) must be demarcated from a PME of the underlying disease. This article explains the differential diagnostic approach and interdisciplinary treatment options are discussed. Menstrual cycle-linked and premenstrual exacerbations in mental diseases necessitate a different approach than a typical PMS or PMDD. A close cooperation between gynecologists and treating psychiatrists is necessary to establish the optimal approach for each individual patient, which can consist of a cycle-modulated psychopharmacotherapy or the adjuvant administration of a hormone preparation.</p>
