Trends in primary malignant brain neoplasms associated mortality among individuals of age 25 years and older in united States from 1999 to 2020: a gender, race and demographics based analysis of CDC wonder database

Between 2014 and 2018, primary malignant brain neoplasms (PMBNs) accounted for 29.1% of all newly diagnosed brain tumors and were the leading cause of cancer-related deaths among males under 40 and females under 20 years of age. This study aimed to evaluate trends in PMBN-related mortality in the United States, stratified by age, sex, race, and geographic region. Mortality data were obtained from the CDC WONDER database for adults aged 25 years and older. Crude and age-adjusted mortality rates (AAMRs) per 100,000 individuals were calculated. Temporal trends were assessed using Joinpoint regression to estimate annual percent change (APC) and average annual percent change (AAPC) in AAMRs. Between 1999 and 2020, 314,347 deaths were recorded due to PMBNs. AAMRs declined significantly until 2006 (APC: −1.4), followed by a gradual increase through 2020 (APC: 0.4). Males had higher AAMRs (8.1) than females (5.3). Among racial groups, non-Hispanic Whites had the highest AAMR (7.5), and non-Hispanic Asians or Pacific Islanders had the lowest (3.0). While most racial groups exhibited a decrease followed by an increase in AAMRs, rates among non-Hispanic Asians or Pacific Islanders remained stable. Regional variation was noted, with the Midwest showing the highest AAMR (6.9) and the Northeast the lowest (6.2). Mortality was greater in non-metropolitan areas (7.5) compared to metropolitan regions (6.7). Nearly half of the deaths (48.9%) occurred at home. Our analysis revealed a downward initial trend in PMBN-related mortality, followed by a gradual increase. Targeted public health strategies are needed to address rising mortality, particularly among males, non-Hispanic Whites, and individuals residing in the Midwest and non-metropolitan areas.
