<p>Drug-resistant tuberculosis (TB) is a significant global challenge, especially in children, where diagnosis and treatment are difficult. This report describes a 4-month-old male infant with pre-extensively drug-resistant tuberculosis (pre-XDR-TB) successfully treated with bedaquiline, delamanid, and contezolid. The patient, born prematurely, developed severe pulmonary TB after exposure to drug-resistant TB from the mother. After discontinuing the initial regimen due to adverse effects, the adjusted regimen resulted in negative TB culture conversion, significant pulmonary improvement, and no notable adverse effects.</p>
