<p>A 17-year-old female with systemic lupus erythematosus developed acute neuropsychiatric symptoms despite improvement in systemic disease activity. Brain MRI revealed bilateral striatal and thalamic hyperintensities on FLAIR, which progressed transiently and subsequently regressed following high-dose corticosteroids, intravenous cyclophosphamide, and plasma exchange. The clinical and radiological features were consistent with neuropsychiatric systemic lupus erythematosus. This case highlights that NPSLE can newly emerge during apparent systemic remission and emphasizes the importance of timely neuroimaging in immunosuppressed patients.</p>
