Introduction <p>The Woven EndoBridge (WEB) device is proven safe and effective for wide-neck aneurysms in GCP trials; simultaneous intra-operative complications have not been reported.</p> Case introduction <p>A 47-year-old man presented with sudden headache; CTA/DSA revealed a ruptured basilar aneurysm, SAH and moyamoya disease. WEB embolisation was planned. An incomplete aneurysm coverage required semi-deployment of a stent-assisted coil, followed by full stent deployment that pushed the WEB into the sac. Micro-thrombi on the WEB protrusion reduced posterior cerebral flow, which was reversed promptly with antiplatelet therapy. Post-op recovery was uneventful; no neurological deficits.</p> Discussion <p>WEB remains safe for wide-neck aneurysms, yet device sizing, release angle and irregular morphology heighten risk. Prolonged manipulation may provoke thrombosis and ischaemia; meticulous technique and rapid antiplatelet rescue are essential.</p>

Introduction
