<p>Moyamoya syndrome (MMS) represents a rare and still under-recognized cause of juvenile ischemic stroke in European populations. We describe the clinical and neuroradiological features of a caucasian adult affected with Trisomy 21 presenting with an ischemic stroke and finally diagnosed with MMS in addition to persistent trigeminal artery.</p>
