Feedback für Rettungsdienstmitarbeitende als Chance für eine bessere Notfallversorgung? Qualitative Interviews

In emergency medical services (EMS), critical decisions regarding treatment and the choice of hospital are made under considerable time pressure. Despite a known high proportion of incorrect working diagnoses, EMS personnel generally do not receive structured feedback from hospitals, e.g., regarding the inpatient diagnoses, diagnostic findings, or the subsequent course of treatment. The aim of this study was to investigate whether and to what extent EMS personnel currently receive feedback on their work, what kind of feedback would be helpful, and how they assess the potential benefits of a structured diagnosis feedback system.
MethodsAs part of a mixed methods study, qualitative expert interviews with EMS personnel were conducted between 12 September 2024 and 8 October 2024 following an online survey. The interviews were conducted based on a semi-structured interview guide and evaluated using deductive–inductive content analysis.
ResultsA total of eight EMS experts participated in two group discussions and two individual interviews. The participants expressed a need for structured feedback, at least on the in-hospital diagnosis, so that they could check whether the preclinical measures taken were appropriate and to further develop their skills. It was emphasized that the introduction of structured diagnostic feedback must be accompanied by the development of an error/safety culture that focuses on opportunities for improvement rather than blame. Implementation was considered challenging due to data protection regulations and heterogeneous structures and technical conditions.
ConclusionAlthough EMS personnel have to make working diagnoses with limited resources and decide on further action on this basis, they currently receive little to no information that would allow them to evaluate the correctness of their actions. However, feedback on the diagnosis alone has the potential to improve, among other things, EMS employee job satisfaction , competence development, and overall patient care. Further studies should explore the feasibility and impact of structured digital feedback systems for inpatient diagnoses.
